1/6/2024 EnchantaVaughn Approved QMHPA arrived to the home of Ms. Brown. Upon arrival to the home, Ms. Brown appeared to be in a positive mood evident by her facial expressions such a smile on the face. QMHPA inquired about her day. She indicated to QMHPA that her day was well. QMHPA informed Ms. Brown that this session was going to focus on her mental health stability. Upon arrival, she did not present any symptoms of S/I. QMHPA began the session by inquiring about Ms. Brown current thoughts, feelings and emotions. QMHPA addressed Mental Health Education with Ms. Brown and engaged her in a discussion on the importance of adhering to her medication on a consistent basis to stabilize her mood and attempt to reduce some of those depressive symptoms she encounters. QMHPA educated Ms. Brown on the symptoms of her Major Depressive disorder and how it can negatively impact her relationships with others. QMHPA continued to encourage and reinforce the importance of identifying the triggers of her stress and depression as well as the importance of verbalizing her thoughts, feelings and emotions rather than isolating herself from others and suppressing her feelings. QMHPA and Ms. Brown reviewed once again the symptoms of Major Depressive Disorder with respect to how it can impact her physically and socially. Ms. Brown appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Ms. Brown was receptive to implementing relaxation and calming skills to help her decrease some of her anxiety. Ms. Brown shared that she does not know how to distinguish between being assertive verses being aggressive. Ms. Brown stated, “People say that I am unstable and I do dumb stuff”. Ms. Brown shared that she feels her life is useless at times but she is never going to give up. Ms. Brown was able to identify that feeling inadequate triggers her depression and her feelings of paranoia triggers her anger. Ms. Brown has agreed to begin thinking positive and filling her day with positive things to do so she can help to decrease some of her depressive symptoms. Ms. Brown has agreed to actively participate in her sessions and be actively involved with her mental health needs.
